Home-based care is the provision of necessary healthcare and personal support services to an individual directly in their home, allowing them to recover, manage a chronic condition, or age with dignity and independence in a familiar, comfortable setting.

This course describes the competencies required to conduct case management. It involves initiating case management process, creating client confidence, identifying client’s problems/needs, developing case management plan, mobilizing case management resources and executing case management plan. It also entails conducting case management follow up and monitoring, evaluating case management outcomes, preparing case management report, disengaging case management process and finalizing or referring case management.

Community empowerment refers to the process of enabling community members to gain control over their lives, resources, decisions, and development processes. It involves strengthening people’s capacities to:
